Telehealth Informed Consent

This Telehealth Informed Consent explains how telehealth services are delivered through SXRX, the benefits and limitations of telehealth care, and your rights as a patient. By using the Platform to obtain medical services, you acknowledge that you have read, understood, and consented to the terms below.

1. Nature of Telehealth

Telehealth involves the use of electronic communications to enable a healthcare provider in one location to deliver clinical services to a patient in another location. Through SXRX, you will exchange information with a licensed physician through asynchronous messaging, intake forms, and (where appropriate) video or phone consultations.

4. Not for Emergencies

The Platform is not intended for medical emergencies. If you are experiencing a medical emergency — including but not limited to chest pain, severe difficulty breathing, severe bleeding, signs of stroke, suicidal thoughts, or any life-threatening symptoms — call 911 or go to the nearest emergency room immediately. For mental health crises, call or text 988 (Suicide & Crisis Lifeline) at any time.

5. Alternatives to Telehealth

You have the right to seek in-person care from a healthcare provider in your community at any time. Telehealth through SXRX is one option among many.

6. Right to Withdraw Consent

You may withdraw your consent to telehealth services at any time without affecting your right to future care. Notify your provider or contact support@sxrx.us to withdraw consent.

7. Privacy and Records

Information shared during telehealth is treated as confidential medical information protected by HIPAA and other applicable privacy laws. See our HIPAA Notice of Privacy Practices and Privacy Policy for details.

8. Prescription Authority

The licensed physician's prescription decision is final. The physician may, at their professional discretion, decline to prescribe medication if it is not clinically appropriate, if you do not meet medical criteria, or if applicable law restricts the prescription via telehealth.

For certain medications (including controlled substances and high-risk medications), federal and state law require us to verify your identity. We use a third-party identity verification provider for this purpose. By proceeding, you authorize this verification.
